The forms on this page should be initiated by an academic advisor as this page is intended for internal use. Forms that indicate "Student" as the initiator, please send the student the student facing page. Appeal/Petition

Grade Option Appeal

University Academic Senate policy requires that registration changes to or from the "Pass/No Pass" option must be processed by the student before the end of the eighth week (one-half of the course). Mini-courses (less than full semester/term in length) and summer session courses follow a pro-rated schedule for "Pass/No Pass" changes.

For appeals regarding the application of this policy, students will need to complete a Grade Option Appeal.

Appeal/Petition

Non-Medical Late Withdrawal

Students may withdraw from all classes, regardless of circumstance, from the first day of classes through the 3/4 point of the term or length of the class whichever is shorter. Any withdrawal after the 3/4 point of the term or class must be for extraordinary circumstances and will be granted only by petition.

If a student is interested in appealing a grade in a College of Journalism and Mass Communications course, they can submit a Grade Appeal.

Appeal/Petition

Request to exceed term credit hour limit

The College of Journalism and Mass Communications limits the number of credit hours a student may take during an academic term. Students requesting to take more hours should discuss their request as well as their specific plan to be successful with their assigned Academic Advisor. Complete this form and submit to your assigned Academic Advisor.

Appeal/Petition

Appeal for Academic Reinstatement

Academic advisors should use the Appeal for Academic Reinstatement when meeting with an undergraduate student who has been dismissed and wishes to file an appeal for academic reinstatement. Appeal/Petition

Emergency Funding Application

Academic advisors should use the emergency funding application to request financial support for (1) tuition and fees or (2) emergency expenses on behalf of students facing emergency circumstances.

Minor/Intercollege Studies Major Substitution/Waiver - Courses for a minor or intercollege studies major

Please complete this substitution form to make a substitution/waiver for a JMC degree-seeking student with a minor or intercollege studies major in a college other than the CoJMC.

Example: A JMC degree-seeking student, majoring in ADPR, needs a substitution in their TMFD minor. The TMFD advisor would need to initiate this form which is then routed to the CoJMC.

Course Override Form

Please complete this form to request an override into a CoJMC course if the course is full, you don't meet the pre-requisites, or the course creates a schedule conflict.

Credit Evaluation

Ed Abroad Pre-Departure (CoJMC Courses)

Upon submission, the form will go to their academic advisor in the CoJMC for initial approval, then will be routed for final approval to the CoJMC evaluator.”  Students will be required to complete the request form and upload a course description and/or syllabus with the request. Upon completion, the request will be routed for approval automatically to the CoJMC evaluator. Students will receive an email after the evaluation is complete. The form will then be routed to Education Abroad and uploaded in the student's MyWorld account.

Credit Evaluation

Ed Abroad Pre-Departure (Non-CoJMC Courses)

For advance evaluation of coursework in departments other than the CoJMC, please download this form to begin the evaluation process. Once the student has completed the form, they should will need to save a copy to their computer and then meet with the designated transfer credit evaluator(s) to review the information. CoJMC students should return the completed form to the director of advising in the CoJMC. Once the form has all the necessary signatures, it will be forwarded to Education Abroad and uploaded to the student's MyWorld account.

Credit Evaluation

International Transfer Credit Evaluation (CoJMC Courses)

Students can request an evaluation for International Transfer Credit for CoJMC courses by initiating this form. Students will be required to complete the request form and upload a course description and/or syllabus with the request. Upon completion, the request will be routed for approval automatically. Students will receive an email after the evaluation is complete. Once the form has all required signatures, it will be routed to Admissions and evaluations will be reflected in the Degree Audit upon processing.

Credit Evaluation

International Transfer Credit Evaluation (Non-CoJMC Courses)

For international credit evaluation of coursework in departments other than the CoJMC, please have the student download the form to begin the evaluation process. Once students complete the form, they will need to save a copy to their computer and then meet with the designated transfer credit evaluator(s) to have the coursework reviewed. Once the form has all the required signatures, it will be routed to Admissions for processing. The Degree Audit should reflect the transfer credit shortly after processing. Domestic Transfer Credit Evaluation (CoJMC Courses)

For domestic transfer credit evaluation for CoJMC courses, initiate this form. Advisors will include information about the student, institution and course for review and approval by the associate dean.
